If you do not receive a 1099-R form by Feb. 16, 2024 or you need a duplicate copy sent to you, please follow the instructions below. Annually after Feb. 16, you may request a duplicate 1099-R online for a prior year.

Retired members may print a report showing their current gross monthly benefit amounts. 

Instructions:

1. Log in to Member Account Access.

2. Select "Forms & Reports" from the navigation bar.

3. Select 1099-R for your retirement claim.

4. Your request will be processed. A duplicate 1099-R form will be mailed to your home address within 7-10 business days.
